Full eligibility criteria
- BMI 35+: Automatically qualifies.
- BMI 30+: Qualifies with heart failure, CKD (stage 3a+), or uncontrolled high blood pressure.
- BMI 27+: Qualifies with prediabetes, a prior heart attack/stroke, or peripheral artery disease.
Who is eligible
You must have Medicare Part D (standalone drug coverage) or a Medicare Advantage plan that includes drug coverage — and you must not already receive a GLP-1 covered by Medicare (for example, for Type 2 diabetes).
